Monday – Friday 10 am – 7 pm | Saturday 12pm – 5pm

Applications submitted within the designated processing hours will be processed on the same day. Applications submitted outside these hours will be processed on the following business day. Please ensure your application includes all required documentation. Incomplete or insufficient documentation may result in processing delays or denial.

Decal request affidavit *

I acknowledge that this form is exclusively for new residents moving in or those who have never before applied for a parking decal. I understand that if I am an existing resident who has previously applied, paid for and received a parking decal, this application fee is non-refundable. Existing residents needing a replacement decal must submit a lost/replacement decal request instead.

Install Instructions: I (1) Peel back to expose the sticky front side of the sticker (2) Place sticker on the inside of the vehicle, in the lower corner of front windshield on the driver's side. Scan the QR code for an image reference Please Note: Do NOT place sticker behind tinted parts of the windshield. NOT installing the sticker properly will result in a citation. ALL VEHICLES THAT DO NOT PROPERLY DISPLAY PARKING PERMIT MAY BE BOOTED OR TOWED AT OWNER'S EXPENSE.

Crime / Property Protection *

Crime / Property Protection I acknowledge and accept responsibility for the conduct of myself and guests while on the property. I understand that if any criminal activity is committed by a guest, Park 360 reserves the right to release vehicle and resident information to the relevant authorities, property management, and any visitors adversely affected by said misconduct or any crime that is committed on the premises.

Mailing Period *

I acknowledge that the decal mailing period is 30-45 days. If I do not receive it within 45 days, I have a 7 day grace period from the 45th day, to contact residentservices@park360pm.com by email. Should I fail to do so within this grace period, I will be required to complete a lost/replacement form and pay the associated fees for a new decal. Decal must be displayed at all times within the mailing grace period.

Resident Responsibility / Guest Parking Addendum *

I acknowledge my sole responsibility for paying and adhering to the parking enforcement of the property and Parking Park360 online portal, or from property management, I understand and accept the violation of parking rules or failure to follow instructions may lead to the booting or towing of vehicles, at the discretion of Park360 LLC, Fraudulent guest parking violations and any fraudulent credit/debit card disputes related to guest parking, initiated by either myself or my guests, will result in the revocation of my resident access to my P360 GuestConnect account.

Fees *

Residential Decal Fee Policy: New Residents only - A one-time courtesy fee of $25 is required for new move-ins to obtain a Residential Decal Lost or Replacement Decals: A fee of $50 will be charged for lost or replacement decals. This fee can be paid through the parking portal. No exceptions will be made to this policy.Reserved/Premier Parking Decal: A fee of $50 will apply for the Reserved/Premier Parking Decal. The fees are non-refundable and cannot be applied towards the Reserved Decal price for additional Premier/Reserved Parking. Residents should ensure that they are familiar with all parking rules and regulations.Please Note: This policy is subject to change. Residents are encouraged to check the parking portal for the most up-to-date information.

Yes | By clicking this option, I have read the policy in the attention portion prior to applying and acknowledge that I approve this charge for an application fee. The Service is as described. Application fees are non-refundable. I accept that the credit is owed, and my card was not stolen, and that my picture ID matches the name on the card used. The following payment is for application fees & whether my application is denied or approved, the fees are non-refundable. I am the authorized user of the card being used.
